Thank you for your interest in Support Tribe, Inc., (“Support Tribe”) and our mobile application Coach On Your Shoulder (the “App”).  Your use of the App is subject to this Privacy Policy. We are pleased to provide this privacy policy to inform you of our practices with respect to how we collect and use personally identifiable information in connection with you and your connection’s use of the App.  

Not Medical Advice

The App enables one-on-one connection through quick daily check-ins and check-ups with your friends and family (your Tribe).  However, our App should not be mistaken for medical advice. 

All content found on the App, including: text, images, audio, or other formats were created for informational purposes only. The content on our App is not intended to be a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition. Never disregard professional medical advice or delay in seeking it because of something you have found on our App.

If you think you may have a medical emergency, call your doctor, go to the emergency department, or call 911 immediately. Reliance on any information found on our App is solely at your own risk. The App and its content are provided on an "as is" basis.

Information We Collect

When you voluntarily provide personal information through the App, we collect personally identifiable information about you, such as your name and email address.  We will not share this information with anyone except as stated in the “Disclosure” section of this Privacy Policy.

Use of Information

Support Tribe uses your personally identifiable information primarily to provide you with the App, to facilitate communication between users, and to provide customized content on the App that is of interest to you and improve the App. The information is also used to verify your authority to access the App and to contact you when reasonably necessary. Support Tribe may also use any information you have provided as reasonably necessary to administer or provide customer support for the App, and to provide you with periodic printed or e-mail communications about new services or upcoming events.   

We also use such information for research and analytical purposes, to respond to your user inquiries, and for other customer service purposes.  

Controlling Your Personal Information

Please do not post any personal information, including any personal health information, on the App that you expect to keep private. You may modify personal information that you have submitted using the App by accessing the App and updating your profile information.

Advertising

Support Tribe prohibits advertisements to be displayed in the App. We are passionate about building a community for our users, and will not sell or otherwise distribute your personal information to advertisers.

Disclosure

Support Tribe will not disclose your personally identifiable information to third parties except as provided below.  If you are not comfortable with Support Tribe’s obligation to share personal information under these circumstances you should not use the App. 

Our App allows you to share personally identifiable information with your friends and family (your “Tribe”).  To provide this functionality, we share the information (as directed by you) with your Tribe. 

Support Tribe will disclose your personally identifiable information as it believes is reasonably necessary to comply with law, regulation or other governmental authority or to prevent harm to yourself or others.  

Support Tribe may also contract with various third parties who help us provide and maintain the App.  For example, we may use a third party to process payments made to us. In these cases, we will use commercially reasonable efforts to prevent such third parties from disclosing your personal information, except for the purpose of providing the services in question.  

Finally, Support Tribe may disclose your personally identifiable information upon a transfer or sale to another entity of all or substantially all of Support Tribe’s stock or assets in Support Tribe’s line of business to which this Privacy Policy relates or upon any bankruptcy or other corporate reorganization.

Only de-identified aggregated information will be used outside of the App.

For your convenience, we may provide links to certain third-party websites or referrals to certain third parties’ products or services.  If you choose to visit a third party’s website or use its products or services, please be aware that the third party’s privacy policy, and not the Support Tribe privacy policy, will govern your activities and any information you disclose while interacting with the third party. 

Security

Because any personally identifiable information you submit to Support Tribe is purely voluntary and should not be of a particularly sensitive nature, we employ our standard security measures with respect to this information and do not use special encryption methods at this time.  Support Tribe user accounts are secured by user-created passwords.  Please note that Support Tribe cannot guarantee the security of user account information.  Unauthorized entry or use, hardware or software failure, and other factors may compromise the security of user information at any time.

It is your responsibility to take appropriate steps to protect against unauthorized access. 

Collection of Information From Children

We do not knowingly collect personally identifiable information relating to children. In the event that we learn that we have collected personally identifiable information from anyone under 13 years of age without prior parental consent, we will take steps to promptly delete such information. By providing your personal information to Support Tribe, you represent that you are 13 years of age or older. 

Policy Changes

Support Tribe reserves the right to change this Privacy Policy, and will post any revisions to our website at www.porchlight.health social media pages, and on our App.  We encourage you to review this Privacy Policy regularly for any changes.  Your continued use of the App will be subject to the then-current privacy policy.

International Visitors and End-Users

At this time, our App is only for United States end-users.  If you are an end-user from another country, please do not submit any personally identifiable information to us our input any personally identifiable information on our App.  

No Error-Free Performance

Support Tribe does not guarantee error-free performance under this Privacy Policy.  We may not always catch an unintended privacy issue, despite our efforts to do so. Accordingly, we welcome your feedback regarding any privacy concerns that you may have, including how we can improve this Privacy Policy.  We will use reasonable efforts to comply with this Privacy Policy and will take prompt corrective action when we learn of any failure to comply with it.

Legal Notices 

This Privacy Policy is subject to the terms in our Terms of Service, available for review at https://www.porchlight.health/terms-of-service/ including terms regarding limitations on damages, disclaimers of warranties, and resolution of disputes.  Our Terms of Service take precedence over any conflicting provision in this Privacy Policy.
